Job Objective
Manage and coordinate the overall care, well-being and daily life needs of Mrs Elisabeth Ferrari, a person affected with fronto-temporal dementia (FTD) that depends, in every sense, on the people around her, so that she’s taken care in the best possible way.
Responsibilities/Duties These include but are not limited to the patient’s:
Personal health care:
Manage and coordinate routine health care habits such as personal cleanliness (showers, manicure, pedicure, hair dressing, clothing etc);
Identify if and when the patient is at risk of hurting herself and take necessary measures;
Plan and monitor the patient’s nutrition;
Medical care:
Manage and coordinates the health care plan by close follow up with the external medical team (local or foreign) that may include doctors, psychiatrists, social workers, therapists, etc.;
Manage and coordinate the health care plan by close follow up the internal team of nurses and lower care givers, making sure the health care plan is followed appropriately.
If necessary (with the help of the patient’s family members if needed), propose the introduction of new members into the network/team of professional and medical staff (external or internal) with the aim of providing the best possible health care plan for the patient;
Communicate and report to family members in regards to the on-going medical treatments and any progress or regressions;
Communicate and report to family members of any change in the medical treatments;
Be aware of the patient’s progress/situation, any new health issues that may arise due to the primary illness or any other reasons and take the necessary actions, including arranging necessary doctor’s appointments etc;
Make sure the environment is as safe as possible for patients with such illnesses;
Prepare, maintain and organize all medical records;
Social care:
Builds relationship with the patient to encourage trust. Listen and interpret needs and concerns;
Plan and coordinate the person’s recreational activities;
Work with individuals, groups and communities to improve the health of the patient;
Plan and organise social events aimed at developing the patient’s social skills and helping to reduce feelings of isolation;
Home management:
Manage purchasing of all home supplies (food, toiletries etc)
Manage home staff (cleaners, cook, guard);
Manage payment of monthly bills (telephone, water, electricity, rent etc);
Manage routine maintenance of the house as may be necessary (electrical, plumbing, painting) to make sure everything is in working order;
Compensation: Compensation will be discussed privately and will be based on the candidate’s qualifications and personal characteristics.
Job Requirement
Personality: Organized, social, caring, empathetic, determined and patient.
Abilities: Organizational and human management skills, as well as good capacity in coordinating and implementing health care plans;
Qualifications: University/college degree in related field of study (nursing, social studies)
Experience: Working experience related to the job description preferably with 5 years minimum experience;
Other requirements: Driving license. Preferred autonomous vehicle that may be used for social care and medical purposes with the patient;
