Background
Maternal and Child Survival Program in collaboration with Oromiya Regional Health Bureau is conducting a study titled “Utilizing All Health System Contacts to Offer Postpartum Family Planning (PPFP) to Pregnant Women and Women within Twelve Months Postpartum in Ethiopia.” As part of the study, the project is looking for qualified and experienced data collectors to collect data from community and health facility levels.

The study will be conducted from February 13, 2017- March 9, 2017 in rural kebeles, health posts, health centers and woreda health offices found in Hitosa and Lode Hitosa woredas of Arsi Zone in Oromiya region.
Specific Tasks:
With guidance from the supervisor, collect data and perform the following as part of the data collection activity:
Deliverables
The Consultant is expected to submit the following deliverables to Jhpiego immediately after end of data collection.
1. Completed questionnaire for clients and facility
2. Reconciliation of any travel advance taken to perform the work within a week after data collection
3. Submission of a Consultant’s Invoice form within two weeks of completion of work
Requirements:
