Carrying out medical data collection and entry activities into the mission database recording all relevant clinical and demographic data on patients and ensuring data quality/reliability, according to MSF protocols and maintaining confidentiality while producing the appropriate, updated documents for the medical team in order to support decision making processes.
Accountabilities
• Performing data entry of all epidemiological information into select databases program and creating appropriate reports, providing data for project development purposes.
• Ensuring that all data entered has been authorized and that appropriate documentation has been received (patient records, treatment monitoring, etc.). Ensuring that all data entry processes occur in a timely and efficient manner.
• Ensuring all data collected and entered to databases is consistently accurate and complete. Being responsible for ensuring that quality control queries of the database are followed up, ensuring identification of data entry errors and corrections. Immediately reporting to the supervisor, any anomalies in the database
• Filing documents according to MSF standards
• Communicating regularly with the HCW’s, assisting in the (re)training when necessary, on proper registration of patients, proper filling of the various forms, clarifying queries, obtaining missing information and correcting errors, ensuring they have ongoing sufficient supplies of necessary hardcopy data collection forms.
Assisting in the compilation and preparation of regular reports and providing detailed information to medical and laboratory teams (weekly, monthly, according to patient treatment requirements).MSF Section/Context Specific Accountabilities:
Main responsibilities and activities
· Ensure collation and entry of all data identified by project medical referent for collection (e.g. morbidity, mortality, surveillance of diseases of epidemics, nutrition, violence related mass casualties, and other disasters, etc) in to the appropriate data management system while ensuring quality of data.
· Implementing and maintaining a surveillance system to monitor epidemiological trends of relevant outbreak-prone diseases. Performing surveillance and/or active case finding, including ensuring understanding of the need for consistent case definitions and data recording.
· Managing data systems and ensuring accurate, high quality databases; analyzing results and preparing regular written reports on progress and findings for project medical referent
· Knowing, implementing and following up the MSF, National, and universal alert and emergency threshold and other protocols for different kinds of scenarios
· Know and ensure proper use of software used for health data recording (i.e. HMIS, REACH, etc.)
· Ensuring high standards quality information /data /Respecting medical secrets and confidentiality at all times.( when necessary)
· Follow and monitor the trends of diseases under public health surveillance through proactive call system,
· Analysis and interpretation of surveillance data and provide report ,regular weekly & monthly feedbacks
· Check to be sure that all questions are asked and the responses are recorded during proactive call
· Follow the instructions of the PMR in order to ensure efficient data collection.
· Administer each proactive call and collect data from each interviewee according to a standard operating procedure.
· Report/share all best practices and challenges to the MERT coordination.
· Work under the supervision/guidance of PMR and Project Coordinator
· Collaborating with Project coordination to influence key decisions
· The ability to travel outside duty station is required.
Data Collection, Entry and reporting
· Collect the relevant data from different departments (including nutrition, EPI, outreach, MCH, IPD, TB, ..) and enter into the MSF data tool.
· Participate in compilation of data both from MSF structures and from collaborating health facilities.
· Ensure that all data forms and patient registration books are complete and accurate before entry. Returning of incomplete or incorrectly filled form, with adequate explanation, for review and completion.
· Participate in the search for missing data.
· Prepare the formats and schedule for data collection to the various departments in the hospital/health centres/ mobile clinics.
· Ensure data extraction to produce monthly/weekly reporting and/or reporting upon request, in different formats (tables, graphics, …), timely and accurate.
· Perform routine checks of medical data against hard copies to ensure reliability of data.
· Maintain a proper filing system of data files, patient records, registers …. according to the guideline, ensuring tidiness and easy accessibility whenever needed.
· Ensure close and efficient collaboration with the concerned departments within the BoH structures.
· Analysis of data,in order to discover anomalies or trends that need further investigation and follow-up.
Information Technology
· Assist medical staff in the understanding and use of the MSF data tool.
· Responsible for data management and use of the data tool: weekly backup, virus updates, report problems …
Other:
· Assist in surveys and operational research projects as requested.
· Participate in medical supervisors meeting and provide when necessary information on data collection, data entry, reporting and trends in the data that require further analysis.
· Manages the MERT medical library by ensuring that all books and kits are well kept, the inventory is updated and a clear borrowing and lending system is installed for medical staff.
· Manages the MERT emergency kits pharmacy ensuring that all kits are well kept, the inventory is updated complet and ready to use, in collaboration with PMR and NTS.
· Receives the MMR feedback and act on it according to the particular needs, after sharing it with the medical team.
